HIPAA Notice of Privacy Practices

This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. SNT Biotech is committed to protecting the privacy of protected health information ("PHI") in accordance with the Health Insurance Portability and Accountability Act ("HIPAA") and applicable state law.

How we may use and disclose PHI

We may use and disclose PHI for the following purposes:

• Treatment: to coordinate and deliver screening and laboratory services, and to share results with your provider and care team.
• Payment: to support billing and reimbursement activities associated with services.
• Health care operations: to support quality measurement, care gap closure, and program administration for health plans, consistent with our agreements.
• As required by law: when disclosure is required by federal, state, or local law.

When we perform services on behalf of a health plan or provider, we typically act as a business associate and handle PHI in accordance with a business associate agreement.

Your rights regarding PHI

• The right to request access to and a copy of your PHI.
• The right to request corrections to your PHI.
• The right to request restrictions on certain uses and disclosures.
• The right to request confidential communications.
• The right to an accounting of certain disclosures.
• The right to a paper copy of this Notice on request.
• The right to file a complaint if you believe your privacy rights have been violated.

Our responsibilities

We are required by law to maintain the privacy and security of PHI, to provide this Notice of our legal duties and privacy practices, to follow the terms of the Notice currently in effect, and to notify affected individuals following a breach of unsecured PHI as required.

Changes to this Notice

We may change this Notice and make the new terms effective for all PHI we maintain. The current Notice will be posted here with its effective date.
